VEI Clinic Apgujeong is an English-friendly internal-medicine clinic providing doctor-led tirzepatide (Mounjaro, a dual agonist) care for type 2 diabetes and medical weight management where appropriate. We prioritize safety, evidence, and clear follow-up—no hype.

Who It May Be For

  • Type 2 diabetes where A1c needs to improve and weight loss would help
  • Overweight/obesity with metabolic risk (e.g., prediabetes, hypertension, fatty liver) where tirzepatide is appropriate under local guidance
  • Patients who prefer a once-weekly injectable with stepwise titration

Not for type 1 diabetes or diabetic ketoacidosis. Not a replacement for lifestyle foundations.

How We Work (Safety-First)

  • Evaluation & labs → A1c, CMP (kidney/liver), lipids (± ApoB), urine albumin/creatinine, pregnancy test if applicable.
  • Medication fit → review history (pancreatitis, gallbladder disease, GI disorders), MEN2/MTC family history, other meds (insulin/sulfonylureas).
  • Titration plan → gradual dose increases to limit GI side effects, with written sick-day rules.
  • Follow-up → early check at 4–6 weeks, then 8–12 weeks with labs; adjust dose/adjunct meds.
  • Lifestyle anchor → protein/fiber targets, zone-2 + strength template, sleep/stress supports.

Typical Titration (Personalized)

  • Start low, then increase about every 4 weeks as tolerated to a maintenance dose.
  • We coach on injection technique, site rotation, and what to do if you miss a dose.
  • If you use insulin or a sulfonylurea, we’ll adjust those doses to reduce hypoglycemia risk.

Common Side Effects & What Helps

  • Nausea, fullness, reflux, constipation/diarrhea → slow titration, small meals, fluids, fiber strategy, anti-nausea plan if needed
  • Injection-site soreness → rotate sites, gentle pressure after injection
  • Possible gallbladder symptoms (upper-right abdominal pain) → tell us early
  • Rapid A1c drops in diabetes can transiently worsen retinopathy → we coordinate eye checks

Serious but rare—seek urgent care: severe abdominal pain (possible pancreatitis), persistent vomiting, black/bloody stool, chest pain, trouble breathing, signs of allergic reaction.

Absolute/relative cautions: personal/family history of medullary thyroid carcinoma (MTC) or MEN2, prior pancreatitis, pregnancy/breastfeeding (generally avoid), severe GI motility disorders. We’ll screen together.

Pre-Op & Travel Notes

  • Procedures/sedation: GLP-1/GIP agents can delay gastric emptying; we’ll give guidance on pausing the medication before your procedure, in line with your surgeon’s or anesthesiologist’s instructions.
  • Travel & storage: store pens as directed on the label (usually refrigerated; limited time at room temperature). We’ll provide a travel letter, packing tips, and a dosing schedule across time zones.

Visit Flow (Typical)

  • Baseline consult (30–45 min) → history/meds, goals, vitals; order labs
  • Results visit (20–30 min) → start/titrate plan, nutrition & training template, monitoring schedule
  • Follow-ups → 4–6 weeks after start/change; then 8–12 weeks with labs (A1c, kidney/electrolytes as indicated)

  • Medication & supplement list (exact doses) and any diabetes data (A1c, glucose meter/CGM)
  • Prior labs/eye exam if you have them

Questions

Is tirzepatide the same as “Mounjaro” or “Zepbound”?

Tirzepatide is the active ingredient. Mounjaro is a brand for type 2 diabetes; some regions use Zepbound branding for weight management. We’ll review what’s available in Korea at your visit.

Can I start at the first visit?

If it’s medically appropriate and your labs are adequate, we can begin with a starter dose and a written titration plan.

Will I need it forever?

Not necessarily. We plan for maintenance, and some patients taper off with strong lifestyle habits. We’ll decide together.

What if I get side effects?

We adjust the pace of dose increases, use meal-timing/portion strategies, and add supportive meds if needed. Most patients improve with time.

Can I switch from another GLP-1?

Often yes—we coordinate washout/timing and choose a safe starting dose.
